I consent to interviews, photography, and/or videotaping and authorize their release, publication, exhibition, or reproduction for public relations, news, education, advertising, research, inclusion on the school website, fundraising, or other school purposes. I authorize the release of personal information to funding agencies for evaluation only.

I waive any right to compensation or royalties and any right to inspect or approve finished materials. I release and discharge the school and its affiliates from liability for any blurring, distortion, alteration, or composite use in connection with these materials. All negatives/positives and media produced are the property of the school or its designee.

I am 18 years of age or older and legally competent to execute this release, or I have obtained the written consent of my parent/guardian. I understand this is a binding legal instrument and I am signing voluntarily.
